SUMMARY OF YOUR RIGHTS UNDER THE FAIR CREDIT REPORTING ACT (FCRA)

ScreenForge Labs LLC, operating HomeProBadge ("HomeProBadge"), may obtain a consumer report about you in connection with your application for a HomeProBadge trust verification. This disclosure is provided in compliance with the Fair Credit Reporting Act, 15 U.S.C. § 1681 et seq.

PERMISSIBLE PURPOSE
You are initiating this verification on yourself to obtain a HomeProBadge trust badge displayed on your public service profile. HomeProBadge obtains consumer reports for trust and eligibility verification purposes in compliance with the FCRA.

WHAT MAY BE INCLUDED
A consumer report obtained for your verification may include:
• Criminal conviction records from federal, state, and county court databases
• Sex offender registry information
• National watchlist and sanctions screening results
• Identity verification results from our biometric identity provider (Didit)

CONSUMER REPORTING AGENCY
Criminal background screening is performed by Data Divers, accessed through the ScreenForge Labs screening proxy. Didit v3 provides biometric identity verification. HomeProBadge does not store raw biometric templates.

YOUR RIGHTS
Under the FCRA, you have the right to:
• Know that a consumer report was obtained about you
• Request a complete copy of any consumer report obtained
• Dispute inaccurate or incomplete information with the consumer reporting agency
• Receive notice before any adverse action is taken based on a consumer report
• Obtain a free copy of your report if adverse action is taken

If you believe information in your report is inaccurate, contact support@homeprobadge.com or the consumer reporting agency identified in any adverse action notice.

AUTHORIZATION
By checking the consent box and proceeding, you authorize HomeProBadge and its designated consumer reporting agency to obtain a consumer report about you for the purpose of verifying your eligibility for the HomeProBadge trust badge.
